- Attested Copies of S.S.C. /Intermediate/Degree Examination certificate(s) must be attached with the form
- Attested Copy of CNIC of the father/guardian must be attached.
- Copy of semesters’ fee voucher
- The Orphan Students will have to supply the Death Certificate of their father issued from the competent authority.
- A pupil’s lawful and defacto guardian may sign income certificate only in case the father is died and certificate to this effect be furnished by the Head of institution.
- The students who have passed their examination by-parts/ in second attempt or improving their division are not eligible.
